Refer to Service Provider

Who can use this feature?

  • Portal Staff who have the Service Provider Group Administration permission to manage Clients.
  • Administrators

The Refer to Service Provider feature allows Portal Staff with the appropriate permissions to transfer a client from one service provider to another. This ensures seamless coordination of services and improves client access to the support they need.

Refer a Client to Another Service Provider

1. Access the Client Profile: Navigate to the Client profile from the People screen.

2. Refer to Service Provider: Click Actions > Refer to Service Provider.

3. Enter Referral Details: Fill in the required and any pertinent optional fields:

  • Service Provider Assignment (required): Select the receiving service provider.
  • Caseworker Assignment (optional): Assign a caseworker at the new service provider.
  • Referral Source (optional): Indicate the reason or source of the referral.
    • If Direct Contact with SSM or Self-Referred (Walk-In) is selected, additional fields will appear to capture the Marketing Sources that contributed to the referral.
  • Reference Number (optional): Add any relevant reference numbers.
  • Referral Notes (optional): Include additional details for the receiving service provider.

Additional Options:

  • Service Needs (optional): Specify any client service requirements.
  • Access to Transportation (optional): Indicate transportation access considerations.

4. Complete the Referral: Click Save to finalize the transfer.

  • Both the client and the new service provider will be notified by email about the transfer.
